If you become injured while doing delivery work, report your claim 24 hours a day, seven days a week.  Just click here to get started online, or call our insurer’s hotline: 833-444-0161 (except injuries sustained prior to 4/01/2021 should be reported to 866-568-2233).  Save the webpage to your favorites and enter the hotline number into your cellphone address book so you have easy access to report a claim at all times.  Also, keep a copy of your certificate of insurance handy, so you can refer back to your policy number.  Make sure to mention you are part of the Association for Delivery Drivers.  A claims representative will guide you through the process and verify certain information about your coverage and the accident.

Unless you complete the online reporting form, you will be asked to submit a claim form, which you can download from this page (see below).  Keep a copy accessible so you can complete it easily and move your claim along.

You should also call the hotline during normal business hours when choosing a physician, clinic, or hospital since there are advantages to using a provider that is part of your insurer’s network.  If your injuries require immediate attention, please go to the nearest hospital for medical treatment or call 911.
