In the past 12 months, has any applicant needed assistance for 2 or more Activities of Daily Living*, received chemotherapy, radiation therapy, or dialysis, or been diagnosed, treated, or prescribed medication by a medical professional for AIDS, AIDS related complex, an immune system disorder, or terminal illness?

Description of Activities of Daily Living: includes bathing, dressing, getting in and out of bed or a chair, walking, using the toilet, and eating.
